CalABA Provider Survey FIN - 2011

This survey is being conducted by CalABA to gather information about the ABA provider pool in California. In turn, CalABA will help its members learn about different organizational models and ways of accessing various funding streams for clients. Survey results will be made available to CalABA members.

This survey is for providers only and only those who are agency owners, senior administrators, or clinical directors.

It will ask you 20 questions about the size of your agency, type of services, supervision practices, and rate models. Assuming you are familiar with these topics, it will take you less than 12 minutes to complete the survey.

You may skip any question you don't want to answer.
