On April 8, 2020, the Public Health Officer issued an Order requiring a one-time disclosure of “Personal Protective Equipment” (“PPE”) and ventilator inventories by all businesses, organizations, non-profits, governmental entities, and individuals. By identifying what resources we already have locally, our community will be less reliant on scarce federal and state supplies, and we can do our part together to keep our doctors, nurses, and first responders safe when treating COVID-19 patients.
Under the law, the information obtained from this survey will remain confidential and will only be used for public health purposes. At the end of the survey, you will find information on how you can donate much-needed PPE and ventilator supplies.
A reminder: only entities and individuals with more than the minimum quantities of PPE noted in the Order and survey are required to fill out this survey.
