CONSENT TO PARTICIPATE
You are invited to participate in this anonymous, online survey that includes questions about the health experiences of LGBTQ+ individuals and the use of tobacco and nicotine products. This survey is sponsored by MashUp!- a Nashville area nonprofit organization whose mission is to connect LGBTQ+ people with the resources they need to thrive. Funding for this survey is provided by the Centers for Disease Control and Prevention (CDC).
The survey should take approximately 10 minutes to complete.
PARTICIPATION
Your participation in this survey is voluntary. You may exit the survey at any time without penalty. You can also skip any question you do not want to answer for any reason.
BENEFITS
You will receive no direct benefits from participating in this research study. However, your responses will help LGBTQ+ community leaders learn more about community needs related to tobacco and nicotine use. This information may help improve services for members of the LGBTQ+ community.
RISKS
The risk to you from participating in this survey is minimal. However, you may find some of the questions to be sensitive or they may cause you discomfort as you think about your experiences. Please discontinue this survey at your discretion.
CONFIDENTIALITY
Your survey answers will be sent to a link at SurveyMonkey.com where data will be stored in a password-protected electronic format. Evaluation team members are the only ones who will have access to your data. Your individual responses will never be shared. No names or identifying information will ever be included in any publications or presentations and your responses to this survey will remain confidential.
At the end of the survey, you will be asked if you are interested in entering your email address to be included in a drawing for a $50 VISA gift card. Your answers will have no bearing on your eligibility for winning the drawing.
