Thank you for considering participation in our Prioritizing Pastoral Care Series on Clergy Well-Being.

The General Board of Higher Education and Ministry (GBHEM) and Wespath are committed to understanding and improving clergy well-being across various dimensions (emotional, social, spiritual, financial, and physical).

Your insights are invaluable in understanding and enhancing the well-being of clergy across The United Methodist Church. Before you proceed, we kindly ask you to review and agree to the following terms regarding your participation and the use of your data:

Purpose of Data Collection: Your responses will contribute to our understanding of systemic impacts on clergy well-being, help us develop targeted support initiatives, and communicate with you.

Data Usage and Sharing: Your responses may be used and shared publicly by GBHEM and Wespath in various mediums, including reports, presentations, websites, print publications, social media, applications for funding, and promotional materials. These responses will not be linked to your name or contact information unless you provide explicit consent.

If you choose to provide your name and email address, they will be used internally by GBHEM or Wespath staff members involved in this project for the purposes of follow-up, additional information, or feedback. This may include invitations to follow-up conversations or meetings.

International Data Transfers: Your data may be transferred to the United States for processing, storage, or further use. The United States laws for the use and protection of personal data may differ from those of your country of residence. We commit to ensuring all data transfers adhere to legal standards and are protected by appropriate safeguards, where appropriate, requiring contractual commitments between companies transferring personal data, as these clauses bind the parties and require them to protect the privacy and security of the data and/or obtaining express consent to the transfer of your personal data.

By consenting to this form, you acknowledge and agree to the transfer of your data to the United States under the terms described herein.

Voluntary Participation and Consent Withdrawal: Your participation in this series is entirely voluntary. If you wish to withdraw your consent after submitting your responses, please contact us at data@gbhem.org, and we will remove your data from our records.
