Caregiver Burden Survey |
Purpose:
We are hoping to learn more about the challenges caregivers encounter as they care for a loved one diagnosed with CCM, and how our programs may evolve to better provide information and support. As a caregiver to a patient with CCM, your voice is critical to help us in these efforts.
In this survey, you will be asked a series of questions about what you may experience in your role as caregiver, your financial situation, household composition, and support systems. All responses will be anonymous. This survey will take about 15 minutes to complete. Surveys must be completed in one sitting in order for the response to be saved. Unfortunately, SurveyMonkey will not save survey progresses to be picked up at a later time.

Data will be collected using SurveyMonkey and analyzed by the Alliance to Cure Cavernous Malformation. SurveyMonkey may use data from the device you use to take the survey (IP address, device type, and internet browser type) to troubleshoot future issues and for service optimization. SurveyMonkey uses technologies like cookies, pixels, web beacons, and page tags to gather information on how you are interacting with the survey webpage. This technology is what allows SurveyMonkey to ensure that a survey is only taken one time per person. They will not use the data collected from your responses to the survey questions.
Participating in this survey is completely voluntary. If you decide to participate, then later change your mind please contact coordinator@alliancetocure.org so your survey responses can be removed from the data file.
