1.Have you been diagnosed with Chronic Kidney Disease by your doctor?(Required.)
2.Have you been specifically diagnosed with Adult Polycystic Kidney Disease (APCKD)?
3.Have you been told that you have high levels of protein in your urine?(Required.)
4.Do you have cysts in your kidneys?
5.Do you know how many cysts you currently have?(Required.)
6.Are you willing and able to have an MRI?(Required.)
7.Have you been diagnosed with Diabetes(Required.)
8.Have you been told by a physician that you have any of the following:(Required.)
9.Do you have a family history of kidney disease?(Required.)
10.Have you had a kidney transplant or the absence of a kidney?(Required.)
11.Are you currently on dialysis?
12.Are currently taking one of the following medications?(Required.)
13.If you are taking any medication from Question 2 (Epogen Procrit, Retacrit or other), how long have you been taking this medication?
14.Do you have a history of any of the following conditions or procedures?
15.Are you currently participating in another clinical trial?
