Before you decide whether or not you wish to participate in this study, it is important for you to understand why the research is being done and what it will involve. Please take the time to read the following information carefully and discuss it with others if you wish.
What is the purpose of this study?
Although there is a large amount of research on the carers in Australia, there is a substantial gap in truly understanding the worth of the carer in Australia presently, as that data does not reach our communities.

This proposed study will fill that gap by generating new knowledge, and presenting and sharing the carer’s lived experience through a documentary film where we can see and hear from a carer directly. This will provide a valuable opportunity to understand how a carer lives. The film will help to reach a wider audience that will include policy bodies, community groups, health sectors, educational settings and the carers' own family and friends, thereby generating new awareness and recognition of the carer in our communities.

Why have I been invited to participate in this study?
This survey has been created as part of the impact study of the research project on the lived experience of carers. As a person who has watched the research project documentary film titled 'Unconditional', you are invited to participate in this project by answering this short survey to share your reflections on the film.

What does this study involve?
If you agree to participate, you are asked to answer this short online survey that will take approximately 4 minutes to complete. You need to undertake this survey shortly after watching the film Unconditional.  The survey will not ask for your personal information other than your age group, gender, ethnicity and state of residence, or country if you are watching it outside of Australia.

Are there risks and benefits to me in taking part in this study?
There will be no benefit or risks for you in participating in this research.

How is this study being paid for?
This study is funded by the researcher's industry partner, Carers ACT, Charles Sturt University, CSU Roberton Scholarship and Charles Sturt University Foundation Trust.

As part of this research impact study, they will both have access to the results of the survey.

Will taking part in this study (or travelling to) cost me anything, and will I be paid?
As it is an online survey, there will be no requirement for travel.  There is no cost to take part in the study, other than your time, and there will not be any payment.
What if I don't want to take part in this study?
Participation in this research is voluntary. Whether or not you decide to participate, is your decision and will not disadvantage you. Only those who give their informed consent will be included in the project.

What if I participate and want to withdraw later?
As this is a survey where the participants are non-identifiable, once the data is completed and submitted, it cannot be withdrawn.

How will my confidentiality be protected?
The questionnaire is anonymous, and it will not be possible to identify you from your answers.

The data will be retained for at least five years at Charles Sturt University as part of the data collection process from the completion of the research project.

What will happen to the information that I give you?
That information collected will be presented as data and will be written in the Researcher’s exegesis for a PhD with Creative Works, for papers in peer reviewed journals, or in presentations of the research study.

As the questionnaire is anonymous, your comments will not be identified in any of the data.
