I accept on my company's behalf the terms and conditions of Gender Fair's Mutual Confidentiality and Non-Disclosure Agreement. I understand that the information shared through this survey, as well as Gender Fair's proprietary methodology and the contents of this survey, fall within the scope of this Mutual NDA.

I attest that all information reported is true, accurate, and current, to the best of my knowledge.

I am authorized and prepared to make the $1,000 Assessment Fee payment at the end of this survey to release my company's initial evaluation. I understand that I must provide an email in order to receive this evaluation, and that processing may take up to two (2) business days.

I understand that the results of this initial evaluation are preliminary and for internal use only, that they may not be shared externally unless I choose to have the reported data validated by Gender Fair. I further understand that Validation incurs a fee of $5,000* and may require the disclosure of internal documents to Gender Fair.
