Chefs' Table - Assessment Tool - Restaurant Employers (Owners/Operators/Managers) Thank you for agreeing to participate! The questionnaire contains a series of questions that will be used to develop a profile of your restaurant and its business practices, which we will use in working with you to identify opportunities for improvement. Any information you provide will be held confidential, shared with our assessor only for the purpose of working with you. The project’s final report will include participant answers in summary form combined with the responses of all other participants in establishing benchmarks for restaurant operations. Your answers will be automatically saved as you go through the survey, each time you go from page to page. You will be able to return to the site to complete the questions and update your responses by using the username and password you create.
