Welcome to the Central Valley's community health survey. The information collected in this survey will allow community organizations serving your county to better understand local health needs. By gathering the voices of community members like you, we can better understand the health challenges the community is facing and can use this information to implement programs that target these needs.
This survey is offered in English, Spanish, Hmong (due to Survey Monkey limitations, you will need to select Lao in order to access the Hmong translation), and Punjabi. Select your language preference at the top right corner of this survey before you begin.
REMINDER: You must be 18 or older to complete this survey. We estimate that it will take 20 minutes to complete. Survey results will be available and shared broadly in the community within the next year. Your responses will remain anonymous and not be associated with your name. Your participation in this survey is completely voluntary. You can skip any question you do not feel comfortable with. At the end of this survey, you can enter into a drawing for gift cards, and more.
