Introduction
The NYS Office of Indigent Legal Services is helping the NYS Commission on Parental Legal Representation examine the quality of free legal representation provided to parents and other adults in Family Court cases ("parental representation"). These include cases involving custody, visitation, paternity, family offense ("domestic violence" or "orders of protection"), child abuse or neglect ("CPS" or "child protective"), foster care, termination of parental rights, adoption, and violation of child support orders.
This survey asks you questions about free legal representation you received in a Family Court case. Your answers are anonymous - they will NOT be shared with your attorney, the other attorney(s) on your case, the judge, or anyone else associated with your case.

The survey will take about ten (10) minutes to finish, depending on how much information you choose to provide. You don't have to answer every question, but the more questions you answer the better we can understand your experience. The survey will be available until November 1, 2018.
